Investigations into the causes of aircraft accidents have for decades focused on what happened and who did it - very rarely 'Why'? It is the question 'Why' that David Beaty has addressed here, fighting the misnomer of 'pilot error' and propounding that the cause should be sought deeper inside human beings who make apparently simple human errors.

Former RAF and airline pilot Beaty (Eagles) investigates planes, pilots, and airline disasters that are commonly categorized under pilot error, a catchall that Beaty finds misleading. Instead, he sees most air accidents as the result of the "collective mistake," where the culprit is rarely one person. Pilots certainly can err, especially when their egos take precedence over standard procedures. But copilots, airline management, air traffic controllers, maintenance crews, aviation designers, and stewardesses can make blunders that lead to horrendous accidents or exacerbate minor ones. Beaty's expert analysis of the events leading up to past catastrophes reveals how seemingly insignificant occurrences can develop into the most terrible outcomes--as evidenced by the 1977 runway collision at Tenerife in the Canary Islands that took 583 lives. Though probably not the book to take on a long flight, this is necessary reading for an informed air-traveling public. Brian McCombie

Hidden gem
By Dean Cannon
Great book. Very glad I took the chance on it. Written in plain speak by a person who has walked the walk. Good psychological insight to the man behind the controls. Some of the reviews made it sound a bit high handed, but as an Englishman myself, I can tell the author is simply writing in his native hand. Some of it is intended to be cheeky. Not the usual red meat and disdain for instances of human error here, but someone clearly trying to understand and explain it.
All good pilots never stop learning and this is a book you will reference back to time and again. I already plan on re-reading it. It is one of those books where you can't get everything on the first pass.

So overwritten that it is not much use to a busy person.
By Jack Bert
I bought this hoping to get a summary of accident investigations. Instead it is diatribe that goes on and on. I should have known from the childish marketing name "Naked".
It does NOT summarize the data and put it in a form that is quickly usable and understandable so that a pilot can quickly learn from it. Does anyone have time to read on and on like this? I read every week much material so I don't have time for "wordy" materials like this. The positive reviews neglect to emphasize this.
A Readers Digest type summary could be written that might be 100 pages long rather than the 300 pages of densely packed material here that I don't have the time to wade through.
For a person writing a report or devoting most of their time to this subject, it is exhaustive in its wondering comments on aviation and why things likely happen. It just spends too much time weaving around for a normally busy person to read it.
Look elsewhere for succinct explanations that you can remember or as rules of thumb for actual situations.

Findings applicable to all industries
By Jezz
This book is a bit dated, but its insights are probably more true than ever.
What the airline industry has found, is that human beings have a hard time keeping up with the technology that they have created. Errors blamed on pilots are usually the culmination point of a long string of errors resulting from commercial and political pressures. The full catalogue of human incompetencies is on display here: engineering errors, cover-ups, dubious management practices and yes, piloting errors.
This is enlightening read for whoever is interested in the interaction between mankind and its technology.
